Your personal Walgreens Health Info Card provides the benefit of...

Your health and prescription information is intended solely for use by health care professionals. Through this program, Walgreens will not intentionally release your particular prescription and health information other than to you, Walgreen Co., and/or your prescriber or other authorized health care professional.
What is the Walgreens Health Info Card?
The Walgreens Health Info Card gives you and health care professionals quick and convenient access to your medical information. It's important to carry the card with you at all times in case of an emergency.
Your attending health care professional can call the toll-free number displayed on the card to speak with our pharmacy staff. We will verify the call and provide the information in your Walgreens Health Info Card file, either over the phone or via fax, including your allergies, emergency contact information and your Walgreens prescription history for the last 15 months.
How does it work?
If your health care professional calls the toll-free number on your Walgreens Health Info Card, a member of our pharmacy staff will record the pertinent information of the requester. We will record the name, location, telephone and fax phone number of the facility, in addition to the requester's title, first and last name. We will then provide them with the medical information you provided for the program.

- to confirm your request for a Walgreens Health Info Card.
- after you update your information and we send you an updated Walgreens Health Info Card.
- if you update your information and a new Walgreens Health Info Card is not necessary.
- when your information has been accessed by a third party.
How does it benefit me?
All of your prescription and health information will be securely stored in one convenient place that you, your doctor or health care professional can access when needed. By carrying the card with you at all times, you'll be better prepared in the case of an emergency. If you are unable to respond or provide the pertinent medical information stored in your Walgreens Health Info Card account, the attending health care professionals can access the medical information about you to help in your treatment. In some cases, this information could be life-saving.
Is there a cost to renew the card after 12 months?
Yes. For only $5, you can renew your Walgreens Health Info Card enrollment. This enrollment is active for 12 months and any updates you make during your enrollment are free.
Other important uses of your card
Your Walgreens Health Info Card gives you 24-hour access to your printable online health and prescription report. This report is a helpful reference for your doctors and health care providers, or for your personal use. Keep a copy of the report at home or your workplace so in case of an emergency, your family, co-worker or paramedics can easily access it.

By registering for the Walgreens Health Info Program, you accept the terms and conditions of the Program as set forth in this website. You also assume the responsibility for ensuring the personal, medical and health information you have provided is accurate and complete. You are responsible for keeping that information up to date and current. For example, if you have had prescriptions filled by anyone other than Walgreens, and you do not supply that prescription information, the prescription history provided to the attending health care professional will be incomplete. It is your responsibility to enter information about such non-Walgreens prescriptions. Walgreens assumes no responsibility for the maintenance or accuracy of the information on the face or back of your Walgreens Health Info Card.
By electing to participate in the Walgreens Health Info Card Program, you authorize and consent to the release of your protected health information (including prescription records kept by Walgreens and any personal, medical or health information provided by you) pursuant to the Walgreens Health Info Card Program procedures.
The Walgreens Health Info Card may not be used outside the United States. The Walgreens Health Info Card Program may be discontinued at any time without notice. Walgreens shall have no liability for unavailability of the Health Info Card Program caused by computer outages, telecommunications outages, software problems or any other cause.
