Are you confused about which Medicare prescription insurance plan to choose? Get a free report with all the information you need to make your choice, including all major plans available in your area and their monthly premiums, co-pays and deductibles.

The free report tells you:

• all the major plans available in your area, including their web addresses and phone numbers
• how many of your current prescriptions each plan includes and which classes and price categories they fall into
• each plan's monthly premium
• each plan's deductible
• the co-pays for each class of prescription

If for any reason you prefer not to use the online report, please feel free to write your prescription medications on our printable list form. Just print out the form, write the names of all your current medications on it, and then bring it to your local Walgreens. The pharmacy staff will be happy to print out a free Medicare Part D report for you.
