I tried a large brace because the package said it would fit a knee measurement of 16 - 18 inches. My knee measures 17 inches with jeans on and I could barely get the brace pulled up (w/o jeans on) and the adjusting straps would no longer reach the loops they're supposed to go through.
The brace actually did help though, but after about 4 hours the side of my knee was geting very sore from the brace rubbing on it. I assume that this was because it was just too tight.
If my local Walgreens carried an extra large, I would definitely try it again.
