4. Download, Complete and Mail the following items to the address listed on your Alcon Official Rebate form. If you don't have access to a computer, call 877-886-4321 to obtain your rebate. This will add two weeks to your processing time.

1. Complete Alcon Official Rebate form.
2. One (1) original package UPC code from product(s) purchased.
3. Original sales receipt indicating total purchase quantity.
4. Copy of original fitting and/or eye exam receipt with fitting and/or eye exam date(s) circled.
