3. Go to lensrebates.alcon.com and enter your individual rebate code which can be found on the rebate slip included with your order.

4. Download, Complete and Mail the following items to the address listed on your Alcon Official Rebate form. If you don't have access to a computer, call 877-886-4321 to obtain your rebate. This will add two weeks to your processing time.

a) Complete Alcon Official Rebate form.

b) One (1) original package UPC code from product(s) purchased.

c) Original sales receipt indicating total purchase quantity.

d) Copy of original fitting and/or eye exam receipt with fitting and/or eye exam date(s) circled.

Other: Valid on purchases December 15, 2012 through July 31, 2013. Rebate submission must be postmarked by August 31, 2013. Product purchases must be made within 90 days of eye exam.
