To qualify:

• No financial requirements*
• Female patient
• New diagnosis of cancer within the past 12 months
• Age 43 years or younger (if above 40 years, AMH should be greater than 0.5)**
• Patient should not have received any gonadotoxic treatment prior to starting a fertility preservation cycle
• Patient will be given a maximum of 2 fertility preservation cycles. The cycles will have to be done prior to gonadotoxic treatment
• Oncologist and reproductive endocrinologist determine that fertility preservation treatment is medically appropriate
• Additional medical information may be required for approval
