HEALTHY ACTIVITIES

• What is 365 Get Healthy Here?

  It’s the Walgreens wellness program and website (walgreens.com/teammember365) for team members and their families. For any questions regarding this program, you can call our Customer Support Center at 1-877-227-3395 or email 365gethealthyhere@walgreens.com.

• Who's eligible to participate in the program?
  • In general, all team members within Walgreens (U.S., PR/USVI, Hawaii) and Duane Reade, including covered spouses/domestic partners enrolled in the applicable company health plan are eligible to participate in the 365 Get Healthy Here program. Due to existing agreements and/or applicable legal requirements, team member benefits may vary and some of the 365 Get Healthy Here program components may not apply to you.
  • Life365 is available to all team members, dependents and other household members. Note: online access to the Life365 digital platform is available to team members only. Spouses, domestic partners, and household members must access services by calling Life365 at 1.855.777.0078.
  • COBRA participants and retirees (pre-65, non-Medicare eligible group only) enrolled in a Walgreens medical plan may also be eligible.
• Is there a deadline to participate and earn any offered incentives?

  Yes, you have until October 31.

• What's in it for me?

  By participating in the program all team members are eligible to earn Walgreens Cash rewards and other perks. Team Members and spouses/partners enrolled in a Walgreens medical plan may also participate in the $0 Rx Copay Program and the Quit Tobacco Program, which provides nicotine replacement patches and gum (and may remove the Walgreens medical plan tobacco surcharge).

• How do I participate?

  Visit walgreens.com/teammember365 on your computer, tablet or smartphone for all the information you need to participate. You’ll receive Walgreens Cash rewards just for registering/logging in.

• Can I choose not to participate in any of these programs?

  Yes. Participation is completely voluntary, however, if you choose not to participate, you will lose out on all incentives and/or surcharge removals as noted.

• What are 365GHH Healthy Activities?

  From January through October 31, all team members, along with their covered spouse/domestic partner are eligible to earn Walgreens Cash rewards and other perks by completing the 365 GHH Healthy Activities, reading information and completing a quiz to test your knowledge.

• How do I find myWalgreens membership number to link to these rewards?

  Login into your www.walgreens.com account, and this will take you to your account home, where your membership number is displayed. Visit www.walgreens.com/mywalgreens to sign up.

• Is my covered spouse/domestic partner also eligible to earn Walgreens Cash rewards?

  Yes

• What if I have multiple myWalgreens account numbers?

  You’ll need to select one account number to earn Walgreens Cash rewards. This will be the account in which the Walgreens Cash rewards are deposited. We cannot reverse the deposit of Walgreens Cash rewards from one card to another, so, it’s important that the correct and same myWalgreens account number is entered every time.

• Do I have to pay tax on the Walgreens Cash rewards?

  Yes, per the Internal Revenue Service (IRS) requirements, when completing 365GHH Healthy Activities, you are responsible for paying tax on the value of the Walgreens Cash rewards you earn. Each $1 Walgreens Cash reward that you earn is taxed at the value of $1. To opt-out, do not complete the 365GHH Healthy Activities and you will not earn Walgreens Cash rewards nor be taxed on them.

• What if I do not receive the Walgreens Cash rewards?

  Please email or call the 365 Get Healthy Here Customer Support Center at 1-877-227-3395.

Walgreens Tobacco-Free Program FAQs

• Where do I access the Walgreens Quit Tobacco Program?

  Log into 365 Get Healthy Here by visiting walgreens.com/teammember365. If it’s your first time visiting, you need to complete a quick Employee Verification process to register. In your Wellness Hub, click the banner for the Tobacco-Free Program. Complete the guided steps to help you reach your Tobacco-Free goals, and if you are covered under a Walgreen’s Medical plan, qualify for Tobacco Surcharge Removal and get free gum or patches (NRTs).

• How do I qualify for the Tobacco Surcharge Removal if I am enrolled in the Walgreens Medical Plan?

  Once the online program is completed, you may qualify for the removal of the Walgreens tobacco-free surcharge if the program is completed within 90 days of eligible plan start date. The surcharge will be removed retroactive to 1/1/19 (for those who enrolled during Open Enrollment) or your plan eligibility date. The refund will be credited via Walgreens payroll. In the instance a team member completes the program after 90 days, they will need to file an appeal through the Benefits Service Center and go through the normal appeal process in order to get surcharges removed retroactive to the eligibility date. Any tobacco changes requested after March 31st of each calendar year and outside the standard correction period following Open Enrollment will need to go through the Walgreens appeal process. Otherwise team members will need to wait until the new enrollment period to declare themselves a non-tobacco user, which will apply for the new plan year. Please note that once the surcharge is successfully removed, a confirmation of coverage is sent to the team member showing the new rates.

$0 Rx Copay Program

• How do I qualify for the $0 Rx Copay Program?

  To qualify for the $0 Rx Copay Program, eligible team members and their covered spouses/partners start by registering for the 365 Get Healthy Here wellness program at walgreens.com/teammember365.Then, complete a 365GHH Healthy Activity (one-quiz question) to qualify for $0 Rx copay.^1,2,3 Please note that $0 Rx Copay eligibility requires annual participation (program expires last day of February of proceeding year). Confirm your $0 Rx Copay status by calling OptumRx at 1-855-376-3214.

• What types of conditions are covered by the $0 Rx Copay Program?

  Eligible conditions include high cholesterol, coronary artery disease, diabetes, high blood pressure (hypertension), asthma and/or weight loss. Note: if the eligibility is on behalf of your covered child, please call OptumRx directly at 1-855-376-3214 and notify them that you are calling on behalf of your covered child.^4,5 To check the Walgreens $0 Rx Copay medication list, visit www.optumrx.com/enroll/walgreens>Forms> Walgreens Zero Copay Flyer. No login is required. Since the medication list updates regularly, please check back each time you need a prescription refill. To be certain your drug is covered, call the toll-free pharmacy number on your member ID card.

• How do I register on walgreens.com/teammember365?

  To register on the site:

  • Enter your own first name and last name
  • Enter the Employee ID of the team member (the primary cardholder for the health plan)
  • For the spouse/domestic partner, you must enter the employee ID of the team member who is the primary cardholder for the medical plan.
  • If you are both Walgreens team members and covered under the same medical plan, enter the EEID of the team member who is the primary cardholder for the medical plan.
• How do I find my Employee ID?
  • For Walgreens team members, your employee ID can be found on your paycheck. You can also visit StoreNet, Walnet or employee.walgreens.com. You will need to authenticate. Then, under “About Me,” select “Payroll,” and click on “Payroll Check Stub.” The employee ID will be located under “General Information.”
  • For COBRA/Retirees (pre-65, non-Medicare), enter your previous Walgreens Employee ID number or call Customer Support at 877-227-3395 to receive a registration ID number.
• How do I ensure my 365GHH and myWalgreens accounts are linked to my team member ID?
  • Add your myWalgreens account number in the myWalgreens tile in People Central.
  • While you’re at it, add your employee ID number to your myWalgreens account for 365 Get Healthy Here earn even more Walgreens Cash rewards.

LIFE365 EMPLOYEE ASSISTANCE PROGRAM (EAP)

• How do I send recognition to my Walgreens coworkers and managers!

  To register on the site:

  • Visit walgreens.com/teammember365.
  • Sign in to your primary Walgreens.com account.
  • Scroll to the bottom of the page and select the Visit Life365 button.
  • Complete your Life365 registration and select Give Recognition at the top of your Life365 news feed.

  You’ll be prompted to select a team member and choose a badge of recognition. Want to recognize a team member but can’t find them in the list? They may not have joined Life365 yet – invite them to do so!
